以“苗”护“苗”，5岁以上学龄儿童疫苗接种提醒通知
The Wisconsin Student Immunization (免疫) Law requires certain immunizations for children in schools. MMSD is cooperating with SSM Health, GHC, UW, and Public Health Madison Dane County to host vaccine clinics for students this winter. Students at age 5+ are eligible for the clinics listed below.
December 14, 2023
10 a.m. to 2 p.m.
Holtzman Building, 333 Holtzman Rd, Madison (park in the back of the building)
December 16, 2023
8 a.m. to 12 noon
Orchard Ridge / Toki Middle School, 5606 Russett Rd, Madison
The following vaccines required by the Wisconsin Student Immunization Law will be available:
DTaP, Polio, MMR, Varicella, Hepatitis B, Tdap(Now a 7th-grade requirement).
In addition, Influenza, Meningococcal, and HPV vaccines will be available and are highly recommended, but not a requirement by the Wisconsin Student Immunization Law.
Please bring your child’s immunization record to your appointment. Contact your healthcare provider or look your child up in the Wisconsin Immunization Registry to review your child’s immunization record and vaccine needs.
